Very Important Note:   This report goes to the Union in a few days. Please fill it out completely and send it promptly. Thank you.

Observations

1. Any delay or omission causes trouble in the preparation of the report the Mission must send to the union. It is very necessary that you send this report, even though every area of the report may not be applicable to your church for that quarter. Every time a report fails to come in, or some data is missing on it, it causes disagreement in the statistics of the Organization.
2. In relation to baptisms/professions of faith for the quarter based upon age, you can secure this information from your Church Clerk. However, it is important that your PM Secretary keeps a record of the number of baptisms that have resulted for the quarter due to direct involvement of Lay members.
3. Under the section captioned Community Outreach, there are two sections. One for Adults and one for the entire church. Usually, the PM report is taken by Sabbath School teachers just before the lesson. This makes up the report for what the Adults do in outreach. The youth are supposed to take a report at AY of what the youth and junior youth are doing. You need to ensure that they follow through on that, as their report will give you a the Total column on what the entire church is doing in relation to Community Outreach.
